To schedule a first time appointment , please call us at 750-0333, Monday through Friday from 8:00am until 5:00pm. Click here to download the new patient information form, print, fill it out completely and bring it with you to your appointment. This will save time at your first visit.

Recare Appointments
If you need to schedule a routine cleaning and check-up, call us at 750-0333 or send an e-mail to appointments@drduckworthdental.com with the days and times you prefer. Our scheduling coordinator will reply with an e-mail for your scheduled appointment. We will do our best to accomodate your time requests.
